We will soon be implementing a form to make the input of your information easy. However, for now please E-mail us with your story. We would like the following information:

• Person treated (optional)
• Person reporting (optional)
• Relationship to person treated (optional)
• Phone (optional)
• E-mail address (optional)
• Age
• Sex
• U.S. state, Canadian province, or country of residence
• What symptoms/conditions you were trying to relieve
• Type of practitioner you visited (e.g., acupuncturist, chiropractor, energy work, body work, homeopathist, iridologist, reflexologist, spiritual work, herbalist, ayurvedic)
• Who recommended this practitioner (e.g., friend, co-worker, advertisement)
• What was promised to you (i.e., what was supposed to be cured)
• The monetary cost of any treatments
• How long the treatment lasted
• What resulted from the treatment (including harmful side effects)
• Any other comments you wish to add
